SUFFOLK, Va. - Virginir -- Lake Prince Woods, a vibrant continuing care retirement community nestled on 172 wooded acres in the heart of Virginia's Hampton Roads area, is proud to announce its new affiliation with Teleios Collaborative Network (TCN). This transition marks a strategic shift from its previous relationship with Senior Options, aligning Lake Prince Woods with a broader network of mission-driven organizations dedicated to excellence in senior care.

Lake Prince Woods has been a trusted name in senior living since 2002. The community offers a full continuum of care—including independent living, assisted living, memory support, skilled nursing, and in-home services through Lake Prince at Home. This CHAP-licensed, Medicare-certified program serves Suffolk and surrounding areas with three specialized services:
  • Home Care: Non-medical assistance with daily living, such as housekeeping, meal prep, and transportation.
  • Home Health: Clinical services including wound care, therapy, and medication management delivered in the comfort of home.
